Medical records have often been referred to as “the source of truth,” and they serve many purposes for veterinary practitioners and their patients. In a perfect world, medical records provide all the details about the patient’s history, clinical findings, diagnostic test results, pre- and postoperative care, prescribed medications, and the patient’s follow up care and progress. When done well, these records can be understood by other veterinary practitioners and will support the veterinarian’s treatment decisions.
